PEVL (Permanent Early Voting List)
Join the Permanent Early Voter List
All voters on the PEVL receive an early ballot by mail approximately 27 days prior to any election in which they are eligible to vote. Getting a ballot early allows time to review the ballot, study the issues, research the candidates and vote at one's convenience. All voted early ballots must be received in the office of the County Recorder, other officer in charge of elections, or deposited at any Ballot Center in the county no later than 7:00 p.m. on Election Day.
Download the form below, fill it out completely, print it, sign it and mail it to the Maricopa County Recorder’s Office at:
Attn: Early Voting
510 S 3rd Avenue
Phoenix, AZ 85003
You can also call us at 602-506-1511 to have one mailed to you.
It is not required to use this specific form to make a request. You can make a written request asking to be added to the PEVL.
The written request must include the voter's name, residence address, mailing address within the county they reside (if different from the residence address), date of birth and the voter's signature.
